Established in 1920 to commemorate the country's participation in World War I.

The medal was awarded to members of the Bulgarian armed forces who served during the war, as well as to civilians who made significant contributions to the war effort. It was also awarded to military personnel from allied countries who fought alongside the Bulgarian armed forces during the war.

The obverse of the medal features the crowned Bulgarian coat of arms, while the reverse bears the inscription "1915-1918" in Bulgarian, surrounded by a wreath of laurel leaves.

The medal was issued in two versions, one with a bronze finish for enlisted personnel and one with a silver finish for officers. It was worn on the left breast, suspended from a ribbon in the national colors of Bulgaria (white, green, and red).
